Pedagogical support plan for premature babies according to the WELCOME IDEA app

The thesis focuses on the area of diagnostics in preschool children in kindergarten in the Hradec Králové region. The main aim of this thesis is to set up a pedagogical support plan, the first level of support measures according to the WELCOME IDEA application for premature children. Another aim is a practical demonstration of how to work with the pedagogical support plan according to the WELCOME IDEA application, analysing its possibilities and limits. Last but not least, the aim is to underline the need of diagnostics in preschool children as a tool for an appropriate pedagogical support plan, which is not only a means for teachers to create optimal learning opportunities for children. The thesis has two parts. In the theoretical part we meet the concepts of family and parenting, psychological needs of parents of premature children, ontogeny of the premature child, psychomotor, communication and socialization in preschool children. The thesis also includes the methodology for the research investigation. The theoretical part (overview of theoretical knowledge) of the bachelor's thesis will present a research of selected key publications on the topic of premature children. The basic facts, specifics, spectrum of deficits or current care for these children will be described, because with the progress of medicine, the needs of premature children with low birth weight have changed significantly over the last thirty years. The work will focus on the knowledge supporting a comprehensive approach to the treatment of premature babies from the time they leave hospital until adulthood. The theoretical part will also include a description of children's coordination skills in the context of developmental kinesiology up to the period of their school age, together with the theoretical basis for the development of these skills. The bachelor thesis also presents a summary of therapeutic procedures for medical rehabilitation, especially from the point of view of physiotherapy. The practical part of the work consists of testing the level of motor functions of a group of 23 premature children with perinatal load aged 9-10 years with criteria: birth before the 32nd gestational week and birth weight less than 1500 g. The Specifics of Speech Development in Premature Infants

The diploma thesis deals with the problems in clinical speech therapy and is entitled Specifics in the speech development of premature infants. The theoretical and practical findings are included in the thesis. The theoretical part is devoted to premature birth, risks associated with premature birth and care of premature individuals. It also describes multiple pregnancy, twin pregnancy and twins in terms of their development, education and specific forms of communication. The last chapter of the theoretical part deals with child development, speech development and communication disorders. In the practical part, the empirical part, the research methods of the qualitative investigation and its focus are described in detail. The main aim of the thesis and sub-objectives are given. The thesis concludes with a summary of the data collected, which was used to develop a grounded theory. The Bachelor's Thesis deals with the issue of premature birth in terms of psychological experience of parents of preterm babies. The work is mainly focused on the mother's psychological coping with this experience. The theoretical part contains the basic theoretical knowledge of preterm baby and its health and psychological specifics as well as the mothers' experiences in the preterm birth event and the relationship between the mother and the baby after the preterm birth. The options of a psychological support for these mothers are also the important part of this work. The empirical part consists of a qualitative research whose main objective is to describe how mothers were experiencing the preterm birth and how they coped with this event. Due to nature of the study the half-structured interview method and the data processing method of the grounded theory were selected. The conclusion compares the theoretical conception of prof. Zdeněk Matějček to the results of my own research.

Caring for High-Risk Newborns after Release from Hospital Treatment: A Parent's Perspective

My Bachelor?s thesis focuses on the issue of children with a development risk, and deals mainly with the care provided to these children after they are released from hospital. The thesis is divided into a theoretical and practical part. The theoretical part defines important used terms. In the Perinatology chapter, general provisions are described as well as the system of perinatologic centres. To understand the topic, it is necessary to know the classification of infants and understand the manifestations and consequences of premature birth, so the second chapter provides basic information about neonatology. A separate chapter deals with the definition of chronological age and corrected age. These are important to secure proper monitoring of the child?s development. The development of prematurely born children differs from the development of children born in the standard term, and this fact must be taken into consideration. Nutrition is an important aspect for proper development. Recent trends especially stress breastfeeding of the child by its own mother. Information about nutrition and breastfeeding and the specifics of nutrition for prematurely born children are described in a separate chapter of the thesis. The following chapter deals with the issues related to releasing children with a development risk from hospital, and the care following their release. The care provided by a development care centre is emphasised. The last chapter provides a selection of the most common diagnoses experienced by prematurely born infants. The practical part contains qualitative research carried out by means of a semi-structured interview. The interviewed sample contains ten mothers of prematurely born children or children with other development risks. Respondents were women that I know, who regularly visit a development care centre with their children. A key selection criterion was the corrected age of the child, which had to be less than 24 months. The goal of the research was to evaluate the activity of the development care centre from the parents? perspective. A single research question was asked: Which deficiencies do the parents see in the activity of the development care centre. The collected data was analysed and lead to a clear conclusion and answer to the research question laid down in the thesis. The results of my research clearly justified the activity of development care centres, which are usually part of large maternity wards. The centres are accepted very positively in all aspects. The interviews also showed some deficiencies, which were described in the discussion and summarized in the conclusion. Since neonatology care is constantly improving, the ratio of surviving children with an extremely low or low birth weight is much higher than in the past. It is necessary to take into account that care does not end after the child is released from hospital. Premature birth carries many risks, which threated the child?s development. It is thus necessary to secure continuous care and monitor the child after release from hospital into home care. This topic has recently been often discussed. The mental development of the child is as important as the child?s physical development. It has been proved that prematurely born children may have some psychological problems. The conclusion summarizes the deficiencies that mothers find in the activities of development care centres, and describes recommendations that should lead to improvement of this care. These include a need for greater engagement of nutrition and breastfeeding counsellors, improved education of nurses employed in community care and improvement of the quality of cooperation with children?s physicians. I would recommend my thesis to the expert and general public. It could also be of use to students of nursing ? midwives, children?s nurses and others who are interested in the issues of children with a development risk.
Health and social issues in children with very low birthweight during the period from hospital discharge to two years of age

The number of new born babies with very low birth weight (VLBW) is increasing. The percentage of those who survive is rising due to developments in modern technologies and science. While dealing with the problem of premature birth the medical, social, psychological and economical aspects needs to be considered. Therefore, the attention is primarily not only on to the question of saving the life but also to ensure the upmost quality of life and health. The care of the child provides a multidisciplinary team, where each role within the team is irreplaceable and their co-operation is necessary to ensure physical and psychological wellbeing of the child. An important role within the team has the surgery of the paediatric practitioner inclusive of the paediatric nurse and a quality follow-up care after the home discharge of the VLBW child. The following objectives were defined in the diploma thesis. To identify how nurses contribute to the care of the VLBW children in the paediatric practitioners surgery. To identify whether nurse given health education is sufficient to families of these children. To find out what are the most common health complications in VLWB children in the paediatric practitioner?s surgery. To find out how VLBW children influence the family psycho-social and economic status. To meet the theses objectives three research question were determined. Which of the nurse?s roles participates most in the care of the VLBW children in the paediatric practitioner?s surgery? Do parents apprehend the field nurse education activity as conductive to solutions to the VLBW issues? Threatens the birth of a VLBW child the family psycho-social and economic stability? The theoretical part of the theses laid out current knowledge in the care of the prematurely born in paediatrics institutions. It stressed out the importance of the elementary presumptions of a successful development such as nutrition, adjusted immunization schedule and differences in multidisciplinary care. Furthermore it explained the nurses? role and outlined the elementary problems premature birth can cause in a family. The practical part focused on qualitative research. The data were gathered via in-depth structural interviews with mothers of VLBW children who reached at the time of the interview 2 years of age and with nurses. The interviews were supplemented with observation. The research results suggest that like mothers like nurses understand the need for a specialized care of the prematurely born children. However, it further suggests differences in their understanding of the nurses? importance and nurses role in this care. The research identified areas which needs improvement in order to provide better care and to improve the mother-nurse relationship in the paediatric practitioner?s surgery. Furthermore, it demonstrated the need for further nurses? education and need for evidence based practice. Moreover, it emphasised the importance of the VLWB children multidisciplinary follow-up care and proved the hypothesis that the birth of a child with VLWB is a great interference with family life, especially in psycho-social domain.
CHOICES OF THE PSYCHO-SOCIAL CARE OF FAMILY WITH THE CHILD BORN PRE-MATURELY

This paper deals with the problems of prematurely born children. Premature birth means an extremely stressing situation not only for the child itself, who is because of its premature birth in danger of many health complications, but also for the child´s parents. The objective of this thesis was to map the possible ways of psycho-social care of a family with a prematurely born child. The work was concieved as a quality research. The data gathering techniques used were semi-standardized interview, participant observation and secondary data analysis. The researched croup were mothers of prematurely born children who gave birth earlier than in the 32nd week of pregnancy and who spent more than three weeks with their children in the infant unit of the hospital in České Budějovice.
